Get proper branch strategy in place, entire dev and QA team should be inline and following branch strategy. Always use separate feature branch for any new feature. Build a plan for testing and merging activity, that needs to be performed by QA only after its validated as meets minimum quality standards. Get a proper regression branch before any new change/feature gets merged to staging.

Defiantly first to identify the problem and debug the particular section of the code thoroughly, perform different testing techniques which give more information about the behavior of the problem also use the users feedback for testing.
