What are the best ways to involve team members in root cause analysis?

In my experience , asking your team for feedback in any form allows your team to feel as though their opinions matter to you as their leader. This can foster a level of trust / buy in from your team members as well!!

Clearly articulating the problem collaboratively builds shared understanding all across: - Gather information from diverse stakeholders to paint a comprehensive picture of the issue from multiple lenses - Host an open discussion where all team members ask clarifying questions & share their unique vantage points--capture their observations visually on a shared whiteboard; facilitating healthy debate at this stage draws out different interpretations of the problem - Once all perspectives are shared, synthesise the viewpoints to generate a problem statement that all agree upon. This statement becomes the "north star" for the RCA - Ensure it is specific enough to be actionable but flexible to revisit, if required

It's best to be more practical than being theoretical. When team members are fully involved in the process, they tend to understand the problem within the process. Therefore, finding solution will be a thing for all, and practical contributions/solutions will be provided by team members.

In my experience first step is to ask identifying what are the problems, which problems generate the highest impact…making the team involved in a transparency exercise is the first activity to make the team feeling part of the improvement.

Before taking the first step, it’s important to have an updated process documentation and have clear understanding of the steps and where is the beginning of the thread. Reverse engineering the process can help isolate the source.

In my experience direct observation is one of the best ways to gather data and insights from the problem you want to solve. Having an “as is” process map in hand while you observe can help you identify deviations and locate hot spots to focus attention.

The best way to do this is with a time line of events and the the supporting conditions for each event listed underneath the associated event. This is important because if you do not know what happened, you cannot determine why it happened. This is commonly known as an Event and Causal Factor Chart or a SnapCharT(R). Once the chart is made, identify the action(s) that, had they not occurred, the incident would not have happened or the consequences would have been mitigated.

The most important part is the data collection. Make sure you consult all involved parties and get their perspective: what happened, under which conditions, why they think it happened, what they would do to prevent it. Don't leave anyone out, no matter how junior. In my opinion, it's a mistake to only gather data about the issue at this stage; you need to gather as much information as possible by asking the right questions. If you have rich data from multiple sources, your analysis will be much more accurate. If you don't have good data, it will just be "garbage in, garbage out".

The tool being used can have a large impact on the ease at which we can collect data. Depending on the problem at hand, selecting a simple and easy tool can help encourage participation from every level. Observation is a great way to get data. However, it does include a flaw, that we observe with our own mental models and experiences as a filter. This is why, I believe, the best data comes directly from the people the run the specific operation in question.

It is very important to correctly identify stakeholders that can contribute to data collection. Another important element is to capture “as is” data to avoid any interpretations at the initial stage.

Collaborative analysis & different perspectives is important: - Review data & look for patterns without pre-assumptions; as themes emerge, dig into asking the "why", to get below the surface - Encourage teams to challenge each other's thinking to avoid bias & draw causal relationships through white-boarding - Keep asking "why" & push teams to keep diving deeper to get to fundamental roots, not just proximate causes & until there are multiple root causes identified & their aligned rationale explaining why each may be an underlying driver - Seek agreement on the root factors based on evidence & data; getting involvement from teams leverages collective critical thinking skills while building buy-in for addressing roots ultimately identified

Brainstorm and collaboration will lead to the right perspective. Look at historical learning’s but also reply on out of box thinking. The 5 why’s is a good tool .

Team members are critical in identifying the root cause because they are on the ground. They can quickly answer the five whys since they usually experience these challenges. Moreover, engaging them in process-solving activities exposes them to understanding the business and provides realistic insights.

Brainstorm, get the ideas, reach a consensus about what the causes are and start making a plan on eradicating them. It is vital your team feels they are valued and can actively participate in problem solving because they will them own it and once resolved male sure they do not happen again

The best way often is to start with what you have, and identify/fill in gaps as you find them. So any RCA should probably use the process SoP as a starting point, then seek to figure out how that can be improved to avoid the problem. Further, the human factor also needs to be considered - sometimes, no matter how well-defined the process is, issues arise due to its implementation or because following the process isn't the easiest thing to do. In such situations, it is important not to place blame or consider it a personal failing, but rather an opportunity to challenge ourselves on how and tool or process can be simplified to reduce the burden on folks performing it, and eliminating problems before they have a chance to arise.

Facilitate brainstorming & encourage thinking big without constraints first. Every idea is valid at this stage-- record them all! - Listing potential solutions, start refinement through constructive debate around feasibility, impact, and resources - Synthesise collective ideas into coherent proposals, providing objective perspective when there is disagreement, & keeping the end goals in sight - Drive consensus around final proposed slate of solutions and secure buy-in by emphasising how each directly addresses the root causes identified earlier through data-driven approach

When looking at solutions you need to facilitate data driven decisions. No solution should be put forward as an option if it does not have clear data behind it, as well as honest data. As you create the solutions, again, involve those affected even if not part of the elements that caused the issue, and create that very clear list of options, their pros and cons, and how they will affect resources, money, timelines of projects, morale, industry perception, and any other positive OR negative impact the solution could create. As I mentioned in a previous answer, people rarely reject that which they helped build, so get all key stakeholders involved!

Ask your team to get the ideas on the whiteboard and discuss the best proposals. At this point you as a leader shoud have significant input on the matter so much so that it should represent the final version of the soultion.

Form cross-functional teams that include individuals with different expertise and perspectives. This diversity can lead to more comprehensive & innovative solutions. Explore various idea generation methods, such as mind mapping, SWOT analysis, or the nominal group technique. These methods can help structure & expand upon ideas. Apply the SCAMPER technique to stimulate creativity. Evaluate how you can Substitute, Combine, Adapt, Modify, Put to another use, Eliminate, or Reverse aspects of the current process or system to address the root causes. consider using the TRIZ method, which is designed to solve complex engineering problems. TRIZ encourages inventive problem-solving by identifying contradictions & inventive principles.

Implementation is where rubber meets road; remove friction & foster teamwork to drive results! - Work with teams to develop an execution plan outlining owners, timelines, resources, risks, & metrics & align solution owners suited to skills, bandwidth, and involvement in the RCA - Coordinate across teams to track progress, rapidly remove roadblocks, & keep momentum-- maintain clear communication on statuses through stand ups & timely reviews - Celebrate successes, coach members through challenges, & model the perseverance required to fully implement changes. By involving the team throughout, we build ownership to see solutions through to completion

Implement the solution can be considered the effective step yet not easy. After analyzing the problem with your team, it is crucial to be able to implement the solution discussed and agreed all together. If that does not happen, credibility could be in question..

You need a clear RACI to efficiently implement solutions and you need frameworks like SMART because one item I believe is critical here is MEASURABLE. You cannot determine success vs completion, if you don't have data points to measure against. Far too often I have seen groups come up with incredible plans, but without ownership of work, auditing of continued completion and quality, and success being measured, then a lot of solutions are half implemented or fail.

Once a " war plan" has been defined then its about doing the right things and double checking the results of the actions taken. Once the results are in its about open discussion on how to change, make them better or make them stick.

I am a huge proponent of lessons learned reports! This really allays the fear of failure, because you become more focused on how to improve rather than who is to blame.

Regular post incident reviews / retrospectives with the team members definitely helps. Documented learnings from past RCA’s can also help reduce frequency/magnitude of similar future incidents.

One thing I have found helpful is creating a guiding team or champions and cascading the change through them. Members of the guiding team are opinion leaders, union members etc. The benefits of these problem solving (what's in for them) is sold to them to get their buy ins. Once this is achieved, its pretty easy sustaining.

In my personal experience the best participative starting point for all RCA efforts would be an agile retrospective dedicated to a challenging team work experience increment. This collective team learning exercise can unearth individual views and contributions about happy co-working experiences, display pain points, and proactively co-involve everybody with suggestions for improvements. This collection of data can be clustered and prioritised so that resolution patterns, methods, tools or formats will organically emerge.

Reflecting on the RCA process and using methods like after-action reviews, SWOT analysis, or lessons learned reports is crucial for continuous improvement and honing problem-solving skills. Involving the team in this final step ensures a collective understanding of the process, allowing for a more comprehensive assessment and learning experience. Their insights and recommendations can significantly contribute to identifying successes, addressing gaps, and planning for future improvements.

The best way to involve team members in root cause analysis is 'Don’t play the blame game!' Root cause analysis is a tool that helps you and your team overcome problems, but it shouldn’t be used to criticize or blame someone. Instead, try to create a blame-free culture, so that your co-workers feel confident in sharing ideas and insights. This will help you to diagnose the issue quickly and effectively.

In my experience I effectively used RCA by using Cause & Effect diagrams/ Ishikawa diagrams in having employees brainstorm the causes to the effect. The brainstorming sessions must welcome all ideas without evaluating them as to whether they are right or wrong. Some use suggestion boxes ?? but I found the Ishikawa diagrams more effective as all members participated freely and felt that their ideas were being valued.

I worked at a company sometimes did meetings to identify why certain projects didn't go well, supposedly in attempt to do better the next time. One colleague who attended these said that all one had to do was "fill the air with words", say something, but management never took the potential lessons learned to heart to solve systemic company wide issues. I joked that one could also "fill the words with air", it was just about the same thing.
