Is safety treated as a routine box-checking exercise, or is it given ample time and attention? Analyze performance evaluations and quarterly reviews - are leaders measured and held accountable for creating a risk-aware culture and implementing safety protocols? If not, the root issue is an organizational lack of focus, rather than individual leadership shortcomings. If the focus on safety isn't something leadership is held accountable for, creating a safety-minded culture simply won't take hold. There has to be a top-down commitment and outcome-based structure in place to achieve this.

The best way to raise awareness of risk is to have experienced it as a disaster. In short, risk culture is as much about the factual in the analysis as it is about the subjective to raise awareness or obtain resources...with nuance between tangible and non-tangible risk (see hacking => subjective > factual in my opinion)
