What is the best way to communicate incident response findings and recommendations to IT management?

Our organisation has a fixed weekly meeting attended by all Engineering Leads. The purpose the is to share learnings from the Root Cause investigation(s) and take away collective actions where appropriate. Format is brief overview of impacts/trigger event and then deeper into what learnings can we share and what actions have/should be taken. We do not cross examine, or lay blame. It’s open, supportive and really focused on what can be taken forward to improve future incident response or, mitigate against a repeat. We also look at thematic themes that may be driving incidents and, successes stories where we have delivered without Incident and how did we do that. More effective than issuing a report to stakeholders (we still do that btw).
