I've found phrases like help me understand, or I think it may have something to do with xx but I could be completely wrong, has a softening effect and open up a more transparent conversation.

It's so easy to review a survey, have one discussion, or see one data point and think you know what root cause is. I tend to use the 5 whys approach. When I make the time to do this, solutions become easily apparent. It's when I skip this step that I typically run into issues. The higher up you go, the more important it is to slow things down because your decision will inevitably have a bigger impact.
