What are the best practices for reporting errors to your development team in Quality Assurance?

- Clear description along with the impact - Actual Vs Expected Outcome - Steps to Reproduce (if applicable) - Setting up the right Priority and Severity of the Bug - Assigning issue to appropriate individual - Wherever possible quantify the impact This should be logged in the Issue tracking tool rather over Email or informal mode. Conducting Defect triage will ensure Defects being monitored and followed up, deciding next plan of action, due dates and much more.

Report what they have done well also. Also, highlight that you have also made mistakes and give examples. Then, go through what they need to improve.

Reporting errors effectively to the development team is crucial for a smooth quality assurance process. Report errors promptly after discovery. Maintain a centralized system or tracking tool to document reported errors,(NCR, INCR, LBE,SOBs, CAR,PAR) their status, resolution, and any additional notes. I could not live without a proper bug tracking system anymore and luckily they have become highly user friendly and cheap. I do however see that proper hygiene is critical in using such system, otherwise it can easily turn into a dumpster. So yes, but do set clear rules for creating and maintaining tickets.

The priority and severity levels of the bugs should be discussed and agreed upon in the session in alignment with the impact of the defect on customers. Representation and expert views from business stakeholders should also be considered in prioritizing the bugs

I find ISO 25010 to be a very convenient reference towards a categorization. What I am use to is to upfront as organization define which characteristics are more or less important and maybe even add characteristics specific to your domain. At reporting a bug, refer to those characteristics (ideally the bug tracking system allows ticking the boxes which are effected). This allows also analysis at some point on what characteristics your product is failing at to take a bigger improvement action.

I would say the practices would include creating the bug with sufficient details to make the developer understand it.Secondly,it should be properly traized.

Especially when errors are detected by means of exploratory manual testing, one should ask the question why the error was not detected earlier e.g. by automated tests. An additional thing to consider then is whether an automated test should be developed and added to catch this error in case of possible regression.

In principle each error is a flaw of the process, whether it is training, unclear process description, unsporting templates, inadequate quality checks, .... That means that each error allows you to learn and improve as organization. It will be a very huge effort to do this for each defect, but for sure for the critical ones you can and should do this root cause analysis towards improving the process.

Ensure it’s a top-down effort: The CEO and board must send the message that the QA process is a priority of the company. Define clear processes: A leader should focus on defining clear objectives, building strong and concise control points/checklists, guiding the team to follow the defined path, doing periodic reviews, and feeling the pulse of the customer throughout. Start the process early in a project life cycle. Set up quality gates at each project phase: During discovery, preparation, exploration, realization, deployment and the run phase.
