To implement a LOTO program, you need to go through six basic steps. Firstly, you should identify the equipment and circuits that need to be locked out and tagged out, review the testing, maintenance, or repair work that needs to be done, and determine the types and locations of energy sources and lockout devices. Notify the affected workers and supervisors about the planned shutdown. Secondly, follow the manufacturer's instructions or standard operating procedures to turn off and isolate the equipment and circuits. Use the appropriate lockout devices to secure the switches, breakers, valves, or other energy sources. Use tagout devices to warn against touching them. Thirdly, test the equipment and circuits to ensure that they are completely isolated and de-energized. Use testing instruments and methods to verify the absence of voltage, current, pressure, or other energy forms. Fourthly, follow testing, maintenance, or repair procedures with appropriate tools and PPE (Personal Protective Equipment). Fifthly, inspect the work area before restoring the equipment and circuits. Check for defects or damages before notifying workers/supervisors that it is ready to be restored. Lastly, document details of LOTO process such as date/time/duration/equipment/circuits/energy sources/lockout devices/tagout devices/workers/supervisors/testing/maintenance/repair work/issues or incidents. Review process for areas for improvement or feedback.
