I have found it useful to create a template as it acts as a checklist to ensure that the BA has thought through all of the potential requirements (example: add a section for NFRs, Reporting, Business Process as well as functional requirements). I have also found it useful to add guidance text for the BA as to how to complete the different sections - for example how to write a user story or when to increment the version number of the document. I mark these in red text to remind me to remove before sending out for wider distribution! It helps new BAs to the team to understand how things are done.

In my mind, this may be the most important aspect to consider when adopting a template (of any sort). Each project, team, and organization is similar in some ways, but unique in many others. A template should be guiding your efforts, not restricting them. In the past, BA's have become “template zombies”, measuring their efforts not by the requirements they have identified, but by the template sections they have completed. That the template is completed is much less relevant than that the information it contains is leading the team to a successful outcome.
