It is fundamental to count with the real experience of each plant. Use the history of process safety incidents of each unit to allow you to analyse with real probabilities. Train operators allowing them to participate in risk analysis and risk assessment exercises, using their real field experience and giving them the possible and potencial real consequences of the safety process risks.

A safe work atmosphere is most important factor . Safe work culture can be implemented by making a strong Emergency Response team from the employee pool covering all areas. So any issues can be highlighted and team need to ensure it’s closed on time . The team should be available round the clock on rotational basis and should effectively communicate with plant EHS team . Need to provide training to all regarding the importance of work place safety

Safety in the workplace starts with changing the culture within your company. You can put up as many safety fences as you want and have your employees wear the right PPE, but if there is no safety culture in the company (top down of course), you will always have to deal with accidents on the work floor. This is probably the biggest challenge for companies as they tend to focus on productivity.

One common mistake workplaces make is when training a new operator, the company asks a seasoned operator to train them. That is horrible mistake because the workplace loses all control over the training process. Because the seasoned operator will unintentionally and unknowingly teach the new operator a shortcut(s). There has to be a written process for training new operators. Workplaces must use the equipment manual as the guide for the seasoned operator to review to the new operator. That will minimize the opportunity for the new operator to learn short cuts.

Conducting thorough risk assessments is essential to identifying hazards with higher probability and greater consequences. This ensures that controls are implemented to reduce risks to ALARP (As Low As Reasonably Practicable). If engineering controls are not feasible, administrative controls must be effectively communicated to teams, with an Emergency Response Plan (ERP) in place.

Según mi experiencia, es importante recalcar a todas las personas que van a realizar tareas dentro de las instalaciones, todos los riegos inherentes al trabajo, cumpliendo y haciendo cumplir la normas de seguridad de la organización como tambien, los lideres del sistema deben estar alertas para asi corregir algún posible desvió minimizando al máximo el riesgo en el trabajo; es inherente generar charlas de 5 minutos reforzando los riesgos a los que estamos expuestos según la actividad a realizar antes de iniciar la jornada, mantener carteles informativos con las reglas de oro implantadas en la organización, para asi mantener a todos los miembros del equipo, la organización y el medio ambiente sin ningún da?o.

In creating an effective reporting tool, it is important to consider its simplicity and ease of use. Once an operator needs to take hours to report a safety concern, he slowly becomes disillusioned to it and we miss an opportunity to correct a safety concern.

In my opinion, training in emergency response is important, but also training for what to do when the responce plan breaks down. If radios fail or responders go down. What if a team member is in lvl A gear and runs out of air and can’t make it out of the hot zone? Does everyone know hand signals if they can’t understand each other trying to talk through a full face respirator? Who is watching for the wind to change and where is the secondary green zone or Decon area if the primary is no longer a safe place? Knowing what to do without having to waste time and confusion is critical.

One thing I've implemented in past operations roles was the roll out of having equipment stored using the lean methodology. Not only does this reduce fatigue but also makes working in an area/unit a lot easier on the operations team. Having boxes of area specific ppe spread about the plant can make a huge difference too.

One thing I found helpful is to consider equipment layout during abnormal conditions of steam outs, winter weather condition and egress/access for emergencies.
