How do you write clear and concise requirements specifications and documentation?

I would consider there to be another step prior to writing requirements. Review and consider the trigger behind the project. Has it come from pain points or opportunities identified by a key stakeholder or driven as part of delivering an overall strategy? What is the 'why' of the project?Challenge the assumption of the nature of the project - have the options been considered (including do nothing)?

I agree with Gill Smith. Is there an under utilized system/process in place that could be adjusted without having to invest in an entirely new product? How does this connect to existing systems? Example - I was once told we had to create spec's for a brand new reporting package. The new system they were considering didn't easily connect to our data warehouse. We spent 3 days researching and found there was a reporting package with the current data warehouse that hadn't been activated and was long forgotten. For much less money and in a shorter amount of time, we brought the reporting package online.

Identifying stakeholders, roles, needs and expectations shouldn't really be an "also" within the scope phase. This is a critical activity in its own right. Maybe a CATWOE perspective for each stakeholder would be helpful. I would also suggest that scope needs to include the environment that that system is going to operate within maybe using something like PESTLE analysis.

Challenge any use of the words "always" or "never" - e.g. "When you say 'never', you mean 'never, never, ever', right'? "When you say 'always' , you mean every single time. no exceptions ever. correct?"

As each requirement is evaluated, consider how a test script could be written such that it proves whether the requirement has been met.

I would add in root cause analysis where the project is aiming to resolve a problem (rather than exploit an opportunity). The analysis will ensure the project is not just addressing symptoms. I agree with Dennis about challenging the words used but I would also ensure that the stakeholders, beneficiaries and others involved in the requirements gathering have not already decided what the solution is or how it should be delivered. Use brainstorming. And consider whether you use Agile/Scrum techniques and start by defining epics rather than full blown requirements

As I reflect on this, I'm reminded of "Coach K"'s philosophy towards coaching which translates into many aspects of life, including project management. Clear communication, accountability/responsibility, and resilience are three elements of his philosophy that resonate for me. My most successful projects had clear timelines and deliverables, collaboratively developed and agreed, with regular monitoring and reporting. This process affords me the opportunity to reallocate resources, provide additional leadership, and coach/develop. It's important to me that the team be resilient to the process by learning from both successes and set backs so that they can apply those learnings to future projects.

"Draw me a picture". If you don't understand what the actual problem is, sit down and go through the pain points live and with capture until you do.
