Then try to check their impact on the process using various statistics tools. Once you find out the factor which affecting the most and then to get root cause issue by various SS tools like pareto chart, why why then you can approach to standard solution to control the error. That requires continue monitor on new process once you satisfied with result then keep continue the same for Controling and proper alert and training to all process owner..

In my experience, the very first thing you should do when something goes wrong in quality assurance is to figure out why it happened. This involves looking closely at the problem, finding out where it started, and understanding what caused it. There are different ways to do this, like asking "why" multiple times until you get to the bottom of it (that's the 5 Whys method), drawing a diagram to see all the possible reasons (like a fishbone diagram), making a chart to see which issues happen most often (that's a Pareto chart), or simply keeping a record of all the errors (an error log). Once you know why something went wrong, you can fix it properly, make sure it doesn't happen again, and stop similar problems in the future.

Prevention also happens through learning from the past. Having a robust and proper closure mechanism facilated by quality team along with delivery, sales and customers will be extremely helpful. By implementing corrective actions, you can solve the issue, make the product or service better, and keep your customers and stakeholders happy.

After identifying the exact root cause the team itself needs to identify the solution as well If that identified solution is a complex one it can be divided into tasks and those tasks can be distributed between team members to implement it. This task distribution will make sure the workload is divided between team members. it will also speed up the implementation of the solution and it will also help to monitor the progress of the solution as well.

The outcomes are highly dependent on how much time and resources are available when dealing with corrective actions. In environments with little processing variation a cause and effect analysis may provide sufficient data points to determine if the measures are effective or not, especially if the cause for defects cannot be replicated. I preferred to use CCC for obvious cases, PDCA for continuous improvement (or when there was only limited resource available) and DMAIC when dealing with complex issues which were not fully understood yet (ie root cause analysis not fully conclusive). In any case, most of the time its a battle against time and resource allocation.

Best industry practises aren't always uniform across industries and regions. The regulatory requirements and obligations varies according to the industries. In my view there are no such thing called quality errors. In the world of Quality Systems we usually deal with product and process deviations which leads to product non conformances and defects.

What I have seen, to avoid and fix QA mistakes, stick to the best practices and rules for QA. Plan out how you'll test things and be clear about what you're aiming for. Use the right methods and tools for your product, like manual or automated testing. Keep talking to your team and others involved, and don't forget to update your process when needed. Following these steps will help you avoid and fix QA errors and improve your skills.

One best practice I've followed & seen the difference is: Shift Left approach. As one of the software testing principles also very well emphasises on: "Early testing saves time & money"

Try to plan and grab as much feedback as you can from different levels: 1. Management - ask them what they think the QA process is and if the result is good. Try to understand if their wishes are useful or not from the best practice perspective. What the business goal? What does the business expect to see at the end of the development phase? 2. Development - ask developers if they communicate enough with QA team or a single QA on the project. Do they know what and how you will test? Do they have a clear bug report? Do they understand the reason of the bug? 3. UAT - as kusers feedback. Is the app's UI/UX is clear? Does the functionality work as they expected? Do they need to make changes or do they wish to see something else?

Continuous learning from feedback is crucial for quality assurance. Learning from feedback will help to prevent and fix QA errors, reduce recurring errors, and improve overall product quality.

A couple of examples of what no need to do in the project: 1. Don't save bugs in comments under the ticket or story. Every issue should be reported and contain as much information as needed. It helps to keep a crystal clear process and don't miss bugs and their priority/severity. 2. Don't ignore developers in STLC. Share with them your test documentation, not only test cases but also test decision tables, transition graphics, pairwise docs, etc. They will use it to develop a better app with fewer bugs.
