How do you involve and engage the process operators and workers in the PHA process?

A process walk down is a critical step in a good PHA and should be led by a knowledgeable, experienced operator. It can help to do this twice - once to define and understand normal operation and a second time to understand potential failure modes and their likelihood/controls in place.

Defining roles and responsibilities is crucial for a successful PHA process. Process operators and workers need a clear understanding of the purpose, scope, and objectives. Assigning specific tasks, providing necessary information, and ensuring ample time and resources are key elements for their effective participation in the PHA.

It's fun to have each core team member "facilitate" a node after spending a few days together, with guidance of course from the official facilitator. Some of then actually do a great job.

An amazing way to put hazards in to perspective is to train around both past incidents applicable to operations or, when available, send HAZOP team members to hazard demonstration full-scale testing. There is nothing like seeing and feeling what actual events are like (from a safe distance)! Also having access to an incident database and hazard testing/demonstration videos on hand when speaking through scenarios can be extremely beneficial.

Process operators & frontline workers can contribute effectively in any PHA if they know the purpose/benefit of their involvement. PHA Leader can provide brief of PHA methodology before start the study to ensure all members onboard with the same understanding. Also to explain why each member’s active contribution is essential for the study’s success. The involvement of frontline workers are such as consideration of all relevant human factors that can create process hazard as well as their required action to prevent/mitigate the consequence to happen. Also the importance of being discipline following procedure as critical aspect to support safe operation. Need to establish trust and maintain it to ensure all voices are heard & considered.

Besides the usual required PSM/PHA training, it is best for operators, with the rest of the PHA Team, to be refreshed on the PHA/HAZOP methodology and concepts through either a separate training or a kick-off calibration meeting on or right before sessions. The primary objective of this kick-off is to emphasize the following: - The WHY of PSM and PHA and really connect the team with the why - Getting to know the each other a bit and break the ice (makes things a bit easier) - How to develop the worst credible consequence (assume no preventive safeguards/they fail). People participating in a PHA for the first time should also get appropriate training, and regular refresher training should be taken into account, especially for infrequent PHA attendees. The person in charge of the other participants' training needs to be the site manager if their position within the company includes overall accountability for the PHA procedure.

Totally agree, those personal are more familiar and close to their facility. Their valuable contributions will really enhance the overall study with real cases and examples. Their should be invited to pre arranged events such as workshops, virtual meetings, conferences and so on. The PHA leader should ask them directly for their perspective on a specific topic pertaining to their area of expertise in order to engage them in the conversation. For example, failure circumstances and their sequence, the efficiency of mitigation or control measures, etc.

Such conduct will incite rivalry among all participating departments and disciplines, which will greatly aid in the identification and location of related hazards as well as the review and enhancement of the process safety analysis study as a whole.

SMART approach to close PHA actions is preferred. The following factors need to be taken into account by the documentation system selected to oversee the actions: Every action has a unique identification number, a set date, and specific objectives for closure. All individuals affected by the PHA, can effectively receive the PHA outcomes, findings, and related actions. Every action's status may be monitored to spot any over due. The authority for action approval is specified. The evidence supporting the closure of the activity is recorded; or, if the approval authority rejects the action, it is noted together with the grounds for rejection and the steps needed to closure. Monitoring and follow-up of process hazard analysis entail regular reviews and assessments to ensure safety measures' effectiveness. Follow ups in my precious experience can be time consuming and challenging, for instance, people or organization changes, priority shift by the business unit etc. However, the specelized HAZOP team eventually play an essential role to keep the ball rolling and catching up with varied parties, especially the leadership buy in. This can be done by Implementing a system that enables continuous monitoring of process variables such level, pressure, and temperature. This will make it possible to quickly respond and prevent any potential problems by detecting abnormalities or deviations from safe operating parameters, In this case the interventions will be enabled and the potential accidents will be prevented.

In order to confirm that potential hazards are identified and the sufficiency of current safety barriers, the PHA team should also review the site's process safety incidents and near misses since the previous PHA, as well as learnings from relevant external incidents for example, from sites using similar processes or technology.
