Metadata should always be up to date with production. If it isn't, it makes your job harder if something goes wrong and you need to track it back to the root cause. You should still monitor in case something slips through the cracks and makes it into production without metadata being documented. However, that should be the exception if you're properly managing your data, including your metadata.
