Although there are some different types of scaffold some things that are the same are: I look at the mudsills and the footings, checking for stability and movement, anchored or secured to the wall. Scaffold planks placed evenly and secured, cross braces, and toe boards, check the ladders. Make sure the scaffold is safe for work required taking into account the weight bearing loads. The scaffold must have a green tag attached once it's assembled and all points inspected to meet requirements.

Industry came up with the 3 colored tag system to assist the user in knowing what the inspection status of the scaffold is. A green tag says that the scaffold is safe to use with no cautions. A yellow tag should be used when there is something that the user should be made aware of to use the scaffold safely. A yellow tag should ALWAYS have something written on it. Otherwise, it is just a green tag that looks yellow. I will not get on a scaffold with a blank yellow tag until I have found out why it does not have green tag. A Red tag indicates that the scaffold is not safe to use.

The best practice starts with the PTW management system for any height work activities along with relevant checklists. PTW audit multiple times, visual inspection, reporting system for the unsafe activities of the workers involved, unsafe conditions of the scaffolding reported & effectively removed, training of the workers, systems to submit observations, competency check of the workers, stability test of the scaffolding, corrective actions promptly taken as priority basis considering risk matrix are the best practices to me that have been followed in my organization.

Competency consists of skills, knowledge, a great attitude, proper training, and experience with basic and/or advanced scaffold structures.

One thing I have found helpful is have there second in charge do a checklist also. Compare yours with the competent person. this way you always have a second pair of eyes.

Scaffolds are very sensitive equipment. In my opinion, after the scaffolding inspector inspects the scaffolding, a senior manager should visit the inspected scaffolds and make a double check. In this way, the effectiveness of the inspections can be seen. For this reason, occupational safety experts and department managers should take this into consideration when inspecting the work done in the field on a daily basis!

In my experience it is better to have a different competent person do the inspections on alternate days. If the same person does the inspections every day they are more likely to get complacent and overlook something whereas fresh eyes will usually see something that is overlooked by someone doing the same inspection day in and day out.

In my experience, I have seen a lot of tag signers but very few actual scaffold inspections. It's rare to see someone who walks a scaffolding level by level and then signed the scaffold inspection tag. Most "Inspectors" walk up look around and sign the tag. Especially if they have multiple scaffolds to inspect pre-shift.
