This could be a simple sketch, wireframe, or storyboard. When you want to involve stakeholders or end users in the design process early on, a low-fidelity prototype can also facilitate open discussions without getting caught up in the details.

The fidelity level of the prototype completely depends on the scale and budget of the project as this will determine the resource available and the timelines. However, I usually prefer medium-fidelity prototypes simply because it is the best of both worlds. It is not so basic that you have to explain everything to the stakeholder, and it is not so advanced that you have already spend time and resource on something that is bound to change.
