Identifying the problem is 90% of solving the problem. I have seen beautiful, elegant, incredible solutions fail because they didn't address the actual issue. The best approach is to go in empty. Check the assumptions at that door and start gathering as many data points as possible. What are the symptoms (i.e. why do I think there is a problem)? What is the process? At each step of the process what can go wrong? Are these issues that contribute to the symptoms? Why and why not? When you have your potential root causes identified, ask yourself what else could it be? Challenge your work, challenge the inherent assumptions that have found a way in. Channel your inner child and ask yourself why so many times that the word loses meaning.

It is also helpful to interview as many people that are involved in the problem situation to get their perspective as to what the root cause is and their perspective as to what they believe possible solutions could be to solve the issue. By doing this, you can avoid bias and supposition.

2 Break down the problem

The next step is to break down the problem into smaller and simpler subproblems that are easier to understand and address. You can use tools like the mind map, the tree diagram, or the SWOT analysis to help you organize and visualize the different aspects, factors, and perspectives of the problem. You should also prioritize the subproblems according to their urgency, importance, and feasibility, and focus on the ones that have the most influence on the outcome.

Breaking down the problem and the contributing process into the smallest chunks possible is critical to truly solving the root cause. This will help you challenge the assumptions you are making that you aren't even aware of and it will help ensure you capture all error prone steps leading to the problem. It's never a good outcome when you solve a problem, but it pops up still due to a second step that was not addressed. Also, be careful as you proceed, you can run into issues with the process that need to be addressed but are not contributing to your problem. Have a methodology to capture those, and include in your solution if possible, but don't get bogged down with additional issues such that you can't solve your for your original aim.

3 Generate solutions

The third step is to generate possible solutions for each subproblem, and evaluate their pros and cons, costs and benefits, risks and opportunities. You can use tools like the brainstorming, the SCAMPER, or the six thinking hats to help you generate creative and diverse ideas, and tools like the decision matrix, the cost-benefit analysis, or the Pareto principle to help you compare and rank them. You should also consider the trade-offs, constraints, and implications of each solution, and how they align with your goals and values.

4 Test solutions

The fourth step is to test the solutions that you have selected, and measure their effectiveness and efficiency. You can use tools like the pilot, the prototype, or the simulation to help you test your solutions in a controlled and realistic environment, and tools like the feedback, the KPIs, or the SMART criteria to help you collect and analyze data and results. You should also monitor and adjust your solutions as needed, and learn from your successes and failures.

In my experience, we also need to consider the emotional aspects of decision making. Often, people's reactions, motivations and biases can affect the process and its outcome. Recognising this, it's important we are empathetic, patient and respectful throughout, acknowledging differing viewpoints. It's also of utmost importance to cultivate a culture that promotes psychological safety, where team members feel safe to voice their thoughts and ideas without fear of negative consequences. These human aspects can greatly influence the success of our problem solving efforts.
