RCA in a nutshell is "WHY". There is always a Human factor. This can almost always be the reason for procedural change. Not only that but it can affect the WAY you train the techs on the procedural change. The real challenge is trying to eliminate your technicians bad habits that might be associated with the old procedure. I feel the only way you get that fixed is by getting your techs to really truly understand the problem and the WHY. Therefore they will actually buy in and WANT to change. In turn the new procedure will be implemented.

2 How to conduct root cause analysis?

Conducting an RCA requires different methods and tools, but they generally follow the same steps. Begin by defining the problem or outcome you want to improve, making sure to use data and evidence to support your definition. Then, collect and analyze data related to the problem, using a variety of sources and methods. Identify the possible causes or factors influencing the problem with techniques such as brainstorming, fishbone diagrams, 5 whys, or fault tree analysis. Evaluate and prioritize these causes or factors based on their impact, frequency, and feasibility. Develop and implement corrective actions to address the root causes or factors using action plans, SMART goals, or PDCA cycles. Finally, monitor and evaluate the results of your corrective actions with indicators, feedback, or audits.

When conducting RCA, it's important to evaluate the effectiveness of any training you have provided or plan to provide to your staff. Training effectiveness is determined by the extent to which objectives and outcomes are achieved and performance and results are improved. To measure this, you need to assess the staff's reaction to the training, how much they learned, any changes in behavior, and the effects on performance and results. To evaluate training effectiveness from RCA, you must define the evaluation criteria and indicators, collect and analyze related data and information before and after the training, and use tools such as SWOT analysis to identify strengths, weaknesses, opportunities, and threats of the training. This will help you visualize and interpret data, as well as evaluate and improve the training.

Monitor and evaluate. First, You need to have a proper and working mechanism in the system that can provide the necessary feedback for Your monitoring acitivites. It can be a report, an e-mailing tool or a weekly sum up message from Your customers. If You still see deviation in the process after the training, than You can start the effective evaluation of the problem and identify what may cause the issue. A training can also be a preventive action however once You see the results are not coming, You need to make a decision and change something in the process. Either in a way of a new training or by introducing an enhanced process.
