1 What is FMEA RPN?

FMEA stands for failure mode and effects analysis, a method that evaluates the potential failures of a process, product, or system, and their causes and consequences. RPN is a numerical score that measures the severity, occurrence, and detection of each failure mode. The higher the RPN, the higher the risk. You can use FMEA RPN to rank the failure modes and focus on the ones that need the most attention and improvement.

2 How to calculate FMEA RPN?

To calculate FMEA RPN, you need to assign a rating from 1 to 10 for each failure mode based on three criteria: severity, occurrence, and detection. Severity is how serious the impact of the failure mode is on the environment, such as pollution, waste, or resource depletion. Occurrence is how often the failure mode happens or is expected to happen. Detection is how easy or difficult it is to identify or prevent the failure mode before it causes harm. Then, you multiply the three ratings to get the RPN. For example, if a failure mode has a severity of 8, an occurrence of 5, and a detection of 4, the RPN is 8 x 5 x 4 = 160.

Be careful in defining your possible values for severity, occurrence, detectability, and the computation to get RPNs. Although it is well known that there are objective ways to determine these values, the subjectivity of the participants still plays a significant role in making the FMEA RPN exercise fail. To minimize this, try to calibrate all participants on the same understanding and definitions. The calibration exercise will allow “defuzzifying” the antecedents that would make the RPN calculation realistic to where it is applied.
