While in requirement / refinement phase, team should finalize UX reviewing it with a UX designer and product owner. Apart from this UX review should be carried out for every small development done on frontend.

This is where way too many skimp; either they genuinely think they really know what their intended market wants or they just don't care ("if we build it they will come" etc.). If you want good market data and aren't in a position to just naturally have it (i.e. you aren't Meta/Alphabet/CIA/etc.) then you really need to invest in getting it. If I'm making a video game targeted at a specific audience then I should be focusing on feedback that I know comes from people who are in that audience. If I'm making web banking interface then...well shit I guess this goes back to what I said before about power users vs everyone else. TL;DR: Anyone you survey should be part of the market you're trying to exploit.
