One if my biggest concerns with the constant drive to efficiency, is the actual understanding of teams needs for well being. This can not be done under the guise of trying to understand if the team is working to best. There needs to be a honest concern for a person not just how well they work.

Here is my concern for this question. QA is not a separate function within the scrum/agile methodology. A story should not be marked as completed until the acceptance criteria is completed and passed. Thinking of QA as a separate function creates a poor scum/agile process.

These are important. The rest are related to the functioning of agile, and you cannot see clear results from them.
