The Best Way to make our Mechanical System secure is by performing the following:- 1. Performing CLITA. CLITA means the symbolic representation of the operation cleaning, Lubrication, inspection, and retightening, if it is done regularly with proper time intervals then breakdown will minimized. When we perform CLIT on our system we get to check the mechanical system in depth. 2. Convert an Open loop system to a closed loop system in mechanical for getting system operation and health feedback. ( Perform LOTO, Install instruments with limits, Implement Fail-safe design closed loop system For Example installing a Proximity sensor for motion sensor which can act as an alarm or feedback of position. 3. Proper Training and OPL( One point lesson)

Few key points that come to mind are: 1. Perform LOTO & restricted access measures 2. Install Instruments & set the alarm points 3. Ensure proper operators training 4. Implement a fail-safe design 5. Keeping a regular stock of recommended spare parts

En mi opinión, aprender de los usuarios del sistema (fundamental) y del historial de incidentes/accidentes a fines de prevenir ocurran nuevamente, en conjunto con una capacitación profunda del personal, tanto jerárquico como operativo. A su vez el equipo de dise?o debe conversar y asesorarse con quienes van a operar el sistema, así tienen todo en cuenta, muchas veces, lamentablemente, solo tienen en cuenta si perspectiva desde un escritorio y no la operativa.
