Safety is treated as a cult dogma, rather than as a practical problem that must be solved with practical effort. This is highly analogous to the way that "Security Theater" in airports reduces security performance. Similarly, safety theater undercuts leadership and demonstrates to the team that the relevant safety measures are not a 'real' concern. And while surface level compliance may be achieved, it will remain an uphill battle, mostly because of the poor leadership.
