It’s important to identify all the stakeholders prior to commencing accident investigations. These may include employees , contractors, line managers, customers, local authorities and government regulators. When setting up an investigation team, representation can be considered from each of these stakeholders. A root cause failure analysis session maybe needed to fully identify what caused the accident and solutions to prevent recurrence. This root cause failure analysis session will require most of these stakeholders who are closely impacted by or involved in the accident to attend in order to get divergent views concerning the accident.

E essa capacidade ocorre quando sabemos que o ser humano é falho e por isso essa capacidade precisa ser mais e mais robusta para que a falha seja segura.

Before involving everyone I. The hierarchy line (from front line to VP) in the actual investigation, I’d ponder what role each person would play and how they’d contribute. I would argue leadership (especially top management) would contribute very little in the actual analysis of the event. Most often they hinder transparency by making front line more uncomfortable in an already stressful situation. That is not “safety culture”. In a culture of care (beyond safety), front line staff supported by a trained professional (eg EHS) can share their stories and identify improvement opportunities. While leadership can support and prioritize resources to conclude action plans.

Engaging external stakeholders should be placed very carefully to reduce any extra charges . If the investigation results were not enough conclusive , Then external stakeholders must be involved from the point where the investigation has inconclusive results . I agree the external stakeholders need to be defined from day one , but should be engaged in a specific stage .

First of all, it is necessary to establish with senior management what is expected of them in the accident investigation process, that they understand that the main role they should play is to guarantee an environment of psychological safety, so that their presence does not bring silence and fear !!! And that they should be involved in the investigation as a speeding-up mechanism, in order to reduce the bureaucracy that normally exists in companies and ensure that actions to improve and generate learning are carried out more quickly !!! And in addition, that it plays the role of encouraging the sharing of the lessons learned at the event throughout the organization !!!
