Mining unstructured data from EHRs is expensive and difficult. Important fields like genomic markers, labs and other items used to measure outcomes should always be available in structured fields that are easy to access and utilize.

Create one medical record that belongs to and is controlled by the patient. This medical record will document their entire medical history from birth to death regardless of the provider or the location where the medical service is provided. This "dense" data will be perfect for use in ML and AI activities.

Create patient data portals that give patient direct access to their electronic health records. I believe when patients have control over their data and can share it with their healthcare providers, it enhances data availability for physicians.

Often too much duplication (often due note bloat) and lack of structure in current data exists. The industry benefits from more standardization in patient data, using technology to narrow the scope in which the provider interacts with data to make it more useful. Patient data is larger than EHRs, but EHR is often the source. Start small by allowing "copy & paste" functionality only if a "modify" action is mandated. Also continue improvements in patient matching/identity promoting quality data. Fixing these 2 problems will make a big impact. I think tools are available, but it takes a great meeting of minds from multiple stakeholders!

As an older clinician, I would rather have the data presented in a meaningful way than trying to “ train” me to interpret the data better. The thought of undergoing more training to deal with systems set up for different purposes than I would want is not appealing. If I wanted to be a tech guru, I would have studied IT not internal medicine.

I think providers need to understand various measure that are called on during audits i.e. HEDIS, etc. A provider’s job is to treat the patient but also to ensure that all their records are intact and secure.

In my opinion, the largest barrier to physician access to patient data is the interoperability of the electronic medical record (EMR) systems housing that data. Potential solutions could include legislating intercommunication between all licensed EMRs. Alternatively, patient-facing medical record trackers can be developed (similar to password managers) that extract patient data into a repository patients can access and then share at their personal discretion.

Integrated and secure health systems with physician friendly electronic medical records data entry platforms will ensure better access.
