In order to reach a real improvment your RCA has to be able to identify a specific cause that can be solved or mitigated therefore your targer cannot be generic but has to be SMART: Specific, Measurable, Achievable, Relevant and Time-bound

2 Choose the right RCA method

There are many RCA methods available, such as the 5 Whys, fishbone diagrams, fault tree analysis, or Pareto charts. Each method has its own advantages and disadvantages, depending on the type, complexity, and frequency of the problem you are facing. When you have identified SMART targets the corrective actions have to be aligned and SMART as well. Note:Remedial actions means actions that targeting the resolution of specific issue instead Corrective actions means actions that reduce or eliminate the root cause in other to avoid the recurrance of the issue

4 Implement and monitor corrective actions

Once you have identified the root causes, you need to take action to eliminate or reduce them. You should prioritize the most critical and feasible causes, and devise corrective actions that address them effectively and efficiently. You should also assign responsibilities, deadlines, and resources for each action, and document them in an action plan. However, your RCA is not complete until you monitor the results of your corrective actions. You should measure and evaluate the impact of your actions on your quality performance, and compare them with your quality improvement goals. You should also track and report any changes, improvements, or issues that arise along the way.

RCA is not only a problem-solving tool, but also a learning opportunity. You should review and reflect on your RCA process and outcomes, and identify what worked well and what did not.
