Correct definition of the problem is key and that must be in business terms. Even IT Ops problems will have a business impact and without an understanding of this prioritisation and downstream activities will be difficult to coordinate. Create hierarchies that build from IT problem to business impact and back down. Adopt an agile, incremental approach to identify, monitor and address each problem to manage impact and ultimately drive improvement.

One of the most critical pieces of this is understanding the data you are using, where it is coming from, what the flaws and assumptions are, and what level of confidence you have in it.
