They are uncomfortable. They think it shows weakness. The most trustworthy leaders realize that mistakes happen and take ownership for them. They acknowledge mistake and use them as opportunities to find ways to improve. They find the broader root cause of mistakes and tie it back to something they could improve as a leader. (e.g., "I should have made the expectations clearer. I should have made sure we had the right training in place.") Good leaders set a tone that acknowledges and learns from mistakes.

People not only look to see if a top executive holds themself accountable, they also look to see if that executive holds their fellow top executives accountable. The failure to address another leader who is a bad apple can destroy a lot of trust and accountability.
