Many companies (mine included) have a very reactive approach to the project life cycle. Unintentionally we often accept flaws will occur and therefore adjust the system to error acceptance. This can lead to a variance (and many times a noticeable one) in project initiation, performance, and execution. This is due to human factor as well but usually paired with inconsistency / lack of clarity in the process / system being (or not being) followed. Start with a wide view and then work inward. The hare company realised its failures in proper planning and execution and potential threats to the entire project at large and took a pause, introspected the strategies and forecast the probable hurdles. After evaluating these speed breakers, they have accepted their greedy speed in execution start and regulated it with timely mannered planning and execution

Failures could be due to a lot of reasons, every experience provides you with a better understanding. Each type of project varies in the way of planning and defining the contractual agreements. Sometimes logistic operations ( disaster with shipping ) and others like a disaster in the production factory on fire or a subcontractor that couldn't adhere to schedules.
