This step is a necessity if you truly want your staff to learn from failures. If you establish that honest mistakes and setbacks aren't part of the normal course of business they will learn nothing but fear and blame. This will lead to dissension among your team and it will make them risk averse. You will lose innovative thinking and openess of brainstorming. In Healthcare, it has been discovered recognizing near misses as they are identified will cause the data to look like you have more errors, but in reality because there is no punishment due honest mistakes, they are more willing to share. The more these findings are shared the better quality can become.

In my opinion, If you’re not failing, you’re not trying. I always say you need to fail forward fast to get where you need to go!

It starts with first accepting that Yes - we failed but not looking at failure as a regret but more like an opportunity where you learned and that will help you in bouncing back even higher. Acceptance removes the fear and blame associated with Failure and helps in more open culture where teams are willing to experiment and try more frequently, fail frequently without being scared and then learn from their mistakes to make something even more memorable.

It is said that Thomas Alva Edison failed more than 1000 times before perfecting the incandescent light bulb, but he learnt from every failure and that shall be spirit. This doesn't mean that everyone shall go through 1000 or more failure, a few are OK if one is learning.

There is much research demonstrating the power of the leader where team members replicate gestures and language and more, unconsciously. Knowing this means a leader must act and communicate in the ways that they want seen in their business.

I try to be clear about our risk appetite, what calculated risks we are happy to take to experiment and innovate. It’s all about mindset and making sure staff feel comfortable to take calculated risks to find the best solutions.

I am incredibly supportive of implementing learning and development programs. I openly share that I have my own leadership and executive coach and love to impart my own learnings to staff that are interested. I strongly encourage learning new skills and strongly support mentorship.

Proposals are treated as negotiable, inviting experimentation and support. Critical feedback is valued as negotiation information, not punished.

When failures occur the tendency is for us to move on quickly, tidy the failure away and concentrate on positive things. We encourage reflection and learning on the things that go wrong without dwelling on them. We treat the cost of our failures as the investment in our learning.
