In my experience is often helpful to have a look at the question if the misunderstanding is caused through bad processes or more on a personal level (did the failure happen again and again? Or was it just once?). Especially in high stress situations it can be helpful to use standard phrases and always recheck with paraphrasing the heard.

Ancestral wisdom says "to err is human, to persist is evil" ("errare humanum est sed perseverare diabolicum"). Don't let yourself caught in the blame game. Instead ask yourself the right questions: - What truly went wrong? - What hint did I miss and why? - If I were in the same situation, what would I ask / check ahead? - If I were in the same situation, what change / action would have yielded a better outcome? - How do I make sure this won't ever happen again? - How can I use this failure as a lesson (to myself) and how can I share it with others/peers? The art of healing is not about protecting bigyet fragile egos, but about sharing knowledge for the purpose of doing no harm.
