One thing I always love is the CQI plan. Does your staff understand it? When they do, and they refer to it all on their own and it becomes a part of the culture, you have a huge win. Failure can be turned into opportunities for growth.

When a project fails, approach it with an open mind, seeking to understand contributing factors without casting judgment. This creates a safe environment for staff to share insights and learn together. Analyzing failures this way helps you identify areas for improvement, innovate solutions, and ultimately strengthen your organization’s resilience and effectiveness.

An organizational culture that both sets high expectations and honors growth through failure has to start at the top. Leaders must open up about their failures and invite their team to join in problem-solving and learning forward. When we do, we set an example that can be practiced by every supervisor and embraced by every employee.

I encourage an environment of “throwing spaghetti at the wall” both in team brainstorming sessions when developing new programs or events, and when we take new projects out into the field. Having a general framework with anticipated outcomes is key (and most often linked to deliverables tied to funding), but how we get there is entirely up to my individual team members so that they can lean into their individual strengths and creative problem-solving, learning and pivoting along the way. This encourages autonomy, leadership development and a deep sense of trust within the team, not to mention some of the most innovative ideas and solutions above and beyond what I or my management team may come up with.

Solid and objective analysis depends on having meaningful and measurable KPIs, which should be determined at the start of a project. What is your objective? What does success look like? Is progress a failure if it doesn't quite get you to the end goal, or is any progress acceptable? To paraphrase the famous philosopher, The Cheshire Cat, "If you don't know where you are going, any road will take you there."

To effectively learn from failures and drive continuous improvement, it's crucial to establish a structured process for evaluation and analysis. Start by scheduling regular debriefing sessions after every major project or initiative, whether successful or not. During these sessions, encourage open and honest discussions, creating a safe environment where team members can share their perspectives without fear of judgment. Assign a facilitator to guide the conversation, ensuring that all voices are heard and that the discussion remains focused on identifying areas for improvement. Document these insights and create an action plan outlining specific steps to address the identified issues and prevent similar occurrences in the future.

Implement Changes: Implement changes to prevent similar failures. For example, after a failed event. Monitor the effectiveness of the changes over time to ensure they are producing the desired results. Learn from Others: Learn from others' failures and incorporate those lessons into your own strategies.
