In my experience I have seen Risk assessment is copy pasted most of the time. Risk assessment should be develop by a competent person who has minimum 5 years of experience on ground. Such has confined space of filter ( vessel) non-phorphorric can't be same as confined space of vessel which has phrophric substance

Per operational expectation, all personnels should be adequately trained in operation and/or maintaining of facilities as this will create awareness of the risks and hazards present in the workplace. Procedures should be duly followed and revalidated occasionally by competent personnel who have had experience with the equipment or facility.

A culture of safety in an organization is the cornerstone of any program. Engagement is key and can be accomplished by listening and involving others in training program development. Determine what works and what hasn’t in the past. Just reading a PowerPoint to a group is not usually effective training. If you know your audience, create good training material and drive engagement, you will be set up for success. You must always also lead by example, never train one way and perform another.

Adequate training is not only essential but a requirement to all employers in creating a safe work place, in how to use a provided PPE, on how to handle harzadeous substances, use of lifting equipment and on how to identify quickly a hazard with in our workplace and knowing how best incidents can be reported report

PPE is sometimes necessary but should always be the last option if other hierarchy of controls cannot be used. Lead by example and obtain a selection of different types of PPE that are comfortable and stylish. Having PPE employees like to wear, solves many issues. When you have PPE options that your coworkers like, there isn’t much need for an enforcement battle. Ask them what they would like.

All kit and equipment must be regularly maintained to a high standard also create maintenance logs service records and create a good housekeeping environment
