Every facility, or organization should have their own, developed safety operations, and mishap reporting procedures. In addition a program such as the "Knock it off" or "Knock it out" programs. Annual review of these documents should be a priority, both as a training guide, and a review for currency. This document should have very clear instructions, something that someone less qualified, than the lowest qualified person in your organization should be able to read and understand. Hierarchy, phone listings, location of safety devices and first aid kits should also be included in this package.

Real open door policies need to be a thing. With a leader who is going to sit there and listen to the concern. The team needs to have trust in the system, and far too often, when these open door policies are used, the member, or the topic is brushed off, leading the team with the mentality of, "nothing is going to change, why say anything?" These feedback loops need to be known that they are only going to be as effective as the leadership team is at communicating, and coming up with a solution to the problem, while not punishing the member or team who brought up the issue.

This is absolutely the case. I used to work in a shop where all the workmanship standards were referenced by posters on the walls. This eliminated the need to pull the book out every time, and allowed for the workmanship standards to remain high. Additionally, allowing quality workmanship guides to be posted, allows for a mentality of quality work to be fostered.

There is a difference between training used as a reminder, and training used for progression. Analysing and preventing unnecessary training can make the team happier, and more efficient. I would challenge to ask, what value does each training requirement add to the member or the team.
