In my experience it is always good to prepare some general questions in advance, even for an investigation interview. You must however not lead the interviewee and be prepared to adapt swiftly with a expending teaching to drill into things. To establish a root cause you could use the '5 Whys', although it is called 5 Why's the number of whys is kind of irrelevant. La mejor información y los datos mas fiables siempre los he recibido del personal que está directamente trabajando en el proceso. Para obtener información fiable siempre he bajado al trabajo de campo. Evita los despachos, hay que meterse en la línea de producción y hablar con los operarios. Entra al laboratorio y pregunta a los técnicos. Acércate al almacén y pasa un rato con los carretilleros. Solo asi conseguiras informacion veraz y sin filtros de la realidad de los procesos productivos.
