I usually start by conducting thorough risk assessments and developing mitigation plans, and mostly keep them as part of my SAD (Solution Architecture Design) document. And embrace a fail-fast approach (my personal favorite approach) to identify and address issues early. Leverage redundancy and modular design to isolate failures and maintain system integrity. Foster a culture of continuous learning and adaptability, ensuring your team is prepared for unexpected challenges. Utilize predictive analytics to anticipate potential risks and make data-driven decisions. This holistic strategy ensures innovation while we continue safeguarding system reliability and performance.

In Railway Systems - if System Reliability is not achieved below mentioned steps shall be followed:- 1- Find more reliable component parts to use. 2- Simplify the design by using fewer components parts, if this is possible without degrading the performance. 3- Apply components derating technique to reduce the failure rates below the average.<international process applied to every component to reduce the opportunity of the component witness more stress that capable to withstand> operating the component below its rated value. 4- If the target system reliability is not achieved even after applying the above 3 techniques.
