Understanding the issue of a product or process is fundamental for engineers. The best way to do this is to deploy some form of root cause analysis to help breakdown the issue. Some good strategies to use KT (Kepner Trigo) Analysis, fishbone analysis or tree diagrams. These methods help sort the noise out of the issue and enable you to drill down on the issue/problem

If you are just starting out in your job, I would suggest getting up to speed on the internal knowledge base of past cases. A lot of products are built off platforms so starting with earlier products when problems were being discovered for the first time usually has the best documentation.

Surprisingly a lot of mistakes get repeated in designs even though the bug was found earlier and it has been carried forward. I think it might be because of poor communication at the company. All that said, it pays to check the previous bugs in past designs to rule it out as an assumption that a well known bug wouldn't carry on in future designs.

I think bell curves are your friend here. What ever statistic you are tracking over the many many samples you have will result in a well distributed curve usually:) Its time to start asking questions if you have an asymmetry to one side. 1) Do you have multiple modes?

If your design teams and product engineering teams don't have an internal wikipedia or knowledge base, definitely create one and document it with lessons learned on this so that it's easier to spread the institutional knowledge to newer members and get them up to speed quicker without tying up senior engineer resources.
