Root cause analysis is a very important tool to prevent accidents from recurrence. many techniques can be used to conduct it including bow tie analysis, SCAT (systematic cause analysis technique), fishbone diagram, 5 whys, FMEA (Failure mood and effect analysis) , FTA (Fault tree analysis), and others. every technique focuses on a specific perspective and can be useful depending on the accident circumstances, but i find the fishbone and scat are the easiest and most applicable in different accidents based on my experience
