Building trust with new physicians: How to do it right?

This is one of the most important issues within healthcare. Studies have shown that poor or mis communication causes many medical errors. From a nursing perspective, always provide information that is pertinent to the situation. This means, before you make the call, consider what questions the physician will ask and get those answers before hand. It will make such a difference when they ask a question and you have the answer already. Ask yourself these questions: what are the vital signs? what medications have been given? what are the most recent labs? What trends am I seeing? Is there a change in the level of consciousness? if so, make sure you have a glucose check and pulse ox check. those are the two most common reasons for LOC change.

When I was in the ICU and a new resident turned to me and said, "what do you want me to do?" be ready with an answer. Do not put them down because they are new. Think about what other physicians did with this type of patient before this one. Consider what you would do. If you are new, as was I, admit that you are unsure. But most nurses have a hint of what is priority with their patients. Whatever is not normal, ask yourself, what can I do to bring that back to normal limits? If it is fast HR: is it fluid deficit, oxygen deficient, pain reaction, stress response? critically think about what is the easiest thing to change first. check for the effect and then try something else if it doesn't work.

Don't be afraid to ask questions. Most physicians are willing to teach you what you don't know. if you are doing a procedure, consider waiting until afterwards. but often talking during a procedure helps allay anxiety all around. they will be appreciative that you considered them approachable enough to ask a question. When I was new in the ICu, i asked the head intensivist why we were giving an iv medication to increase the BP and one to decrease the BP at the same time? He was pleased as punch to tell me a long lecture on afterload and preload on the heart and how the inotropes worked. more than I expected, but I had a good lesson. Ask questions.

Ask them about themselves. Most people are willing share something about themselves that may be less personal. such as what school they went to for medicine. why did they go there? What specialty are they thinking of? Where would they like to finally end up location wise? All of these show that you are interested in them. That helps to build a friendly atmosphere.Share some corny jokes with them. It will make their day. Watch out for neuro surgeons. they are the one group I had difficulty with. Many are reserved and closed people.
