I like using this mantra - We Think, We Can and most importantly We WILL accomplish our customers' requests. With that being said yes parameters to Scope of project and details lying withing the scope need to be addressed, prioritized as well as communicated effectively in charter to ensure all stakeholders and business entities collaborate on what is necessary, efficient, value added and sustainable as well and where necessary make amicable adjustments for successful project delivery.

It requires a definite project planning. Obviously last minute changes from the client are inevitable. What we can do is that in project planning phase we always need to keep a buffer time , such that if any changes come, it can be completed within the buffer time.
