It's a questionaire where the person being evaluated includes his self-evaluation and ask other colleagues to complete the information to get their feedback on how they would present him in a professional context, 3 competencies he stands out and his 3 improvement points. It also includes a quantitative evaluation in several fields and potential limitations that may affect his professional career. The results allow awareness. And it's possible to use this information to set up an action plan.
