Understanding Equity vs. Equality Equity involves providing care tailored to the individual needs of patients, acknowledging that different patients may require different levels of support based on their unique circumstances. Equality, on the other hand, implies offering the same resources to every patient, which can perpetuate disparities rather than resolve them

Navigating limited resources in patient care requires a strategic and patient-centered approach, and advocacy engagement plays a crucial role in ensuring equal attention to all patients. By actively collaborating with advocacy groups, we’re able to gather insights into the unique needs of different patient populations. This allows us to prioritize our efforts based on the most pressing needs and ensure that no group is overlooked. In practice, we work closely with these advocacy organizations to identify gaps in care, understand the social determinants that affect different communities, and create targeted solutions that address those disparities.

It’s important to assess each patient’s needs, ensuring urgent cases are addressed first while no one is overlooked. Using technology, like care management systems, helps track patient progress and ensures care is distributed evenly, preventing any gaps in attention. Open discussions with patients and families about care priorities build trust and set realistic expectations. By promoting a well-coordinated team approach, we can pool our strengths to fill any resource gaps and provide consistent care for all.

A host of resources are becoming available through CMS/HHS as a result of digital innovations and the need to increase equity and achieve quality outcomes while reducing 1) costs and 2) physician and staff burnout. Specifically, reimbursable CPT codes for chronic disease management and remote monitoring of patients AFTER they leave the acute care setting or physician practice. Many studies have shown that these programs can help keep patients engaged, improve satisfaction, and improve outcomes through continuous, equitable, and reimbursable engagement.
