In my experience, most safety-critical conditions are those in which humans can intervene in equipment that is moving or has the potential to move. Even if we know it is dangerous, we can put our hands or bodies into equipment due to a sudden decision or mistake, and it is necessary to design for every possible risk so that no matter what a human does, we will never get hurt. In addition, if the current facilities are not based on such a philosophy, management should take the initiative to improve them.
