Governance and communication plan is key. create a RACI matrix with clear roles, accountability and responsibilities and share it with the team. weekly, monthly and QBR reporting with necessary stakeholders are must. Outcome and Action plans out of these meeting need to be actioned.

My solution to getting everyone on the same page, is to employ the tool of effective project management. The leading global association for the project management profession, which is the Project Management Institute or the PMI, defines a project as a temporary endeavor undertaken to deliver a specific project, service or result. One problem with professionals is that every time they hear the word "project", they think of something big with a big budget, so I have invented a definition of project. Every time you set out to accomplish something specific and unique within a given time frame whether your budget is $1 or a billion dollars, a project is initiated.
