The biggest no-no I keep seeing across the entire industry, is when there is a presence of shared work computers or even shared users, the frequency that one experiences the giant security hole of a password scribbled on a post-it note is beyond me. Worse still is when there are policies in place for changing passwords every x time. people using the same arbitrary word followed by an increasing Integer. Ie. Password1 then Password2 etc. Under these conditions, all a bad actor would have to do is take a photo and they are golden.
