This is very web centric but all libraries expose APIs as well. I think APIs need a usage documentation which is more tutorial/example-based and of course the API reference showing the details.

Using existing tools for generating documentation is cumbersome in my opinion, since usually you want to highlight multiple aspects of a project: the domain, the API contract, error cases, business flows, design decisions, business context, etc. Each tool will only cover one aspect and output in it's own design flavour. No solo es importante elaborar una buena documentación, hay que estructurarla muy bien para que pueda ser entendida por todo el mundo. Y lo mismo pasa con el código. Es más gratificante ver un código lo más limpio y legible, y además bien documentado.
