First, realize that everyone up there is nervous. I've given 1000+ keynotes, and I'm still scared before I go on. Second, be transparent. If you are in the grips of true stage fright, tell the audience. Just saying it out loud will release a lot of your own tension as well. Third, work with what you have. You're there to get through it, and if you do that, you've done better than 97% of people. Trust me, as long as you don't faint on stage, most people in the audience will still have found things to enjoy in your speech. If you do faint, you may have the seeds of a really interesting TED-talk, as long as you can work up some lessons from it, such as "What fainting during a keynote taught me about resilience". Heh.
