Understanding the root cause helps you fix the real problem. 2. Revisit Goals & Expectations: Were the goals for automation realistic? Did you underestimate complexities? Realigning goals and expectations prevents future oversights. 3. Data & Measurement: Did you track progress? Analyze data to see where automation fell short. Data helps identify areas for improvement. 4. Employee Feedback: Did employees resist change? Gather feedback to understand their concerns and address them in future projects. 5. Continuous Improvement: Automation is a journey.

With the experience I’ve had, first thing is to identify the root cause by implying the 5why analysis that will help you eliminate the problem for future failures

For an error to be corrected the foremost requirement is acknowledging that there exists an error. Next logical step would be to break down the entire process in segments to understand what went wrong and where. .

Avoiding careless mistakes at work is crucial for maintaining productivity and achieving success. By implementing simple strategies such as taking breaks, double-checking work, and staying organized, you can significantly reduce the possibility of making mistakes.

For me I think it is important to conduct a Post-Mortem Analysis and focus on what went wrong and them how we can improve the situation: This will include the following 1) Bring together all stakeholders involved in the project to discuss what went wrong. 2) Use techniques like the Five Whys or Fishbone Diagram to drill down into the root causes of the setbacks. 3) Create a detailed report outlining the issues, root causes, and potential solutions.

Human Being & Machine don't go hand in hand. Upon identification of the root cause, a holistic review and brain storming needs to be done for corrective action required. Agreed change needs to be run as a standalone pilot. This will help identify gaps and / or any fine-tuning required.

Revising your automation plans is crucial after identifying failures. Start by reassessing the tools and workflows you initially chose. Ensure they align with your business objectives. Provide additional training for your team where necessary. Document every change thoroughly to maintain clarity and track progress. This process prepares your business for more robust and successful automation, setting a strong foundation for long-term efficiency and effectiveness.

Feedback should be intentional, and writing it down first encourages you to understand your intention. It also allows you to evaluate your feedback for clarity, (3) Regular feedback is important. Busy days or frequent work tasks can make it challenging to maintain routine feedback.

Thorough testing is crucial for preventing automation failures. Start with comprehensive unit tests for all components and conduct integration tests to ensure smooth system interactions. Perform end-to-end tests to validate entire processes, and use stress tests to identify performance bottlenecks. Involve end-users in user acceptance testing (UAT) to verify usability. Implement automated regression testing for ongoing quality assurance and use scenario-based tests to cover different use cases. Conduct security tests to identify vulnerabilities and test with real-world data for practical functionality. Establish a robust QA process with clear criteria and continuously refine test cases as processes evolve.
