Safety adherence is impracticable without motivation. A healthy discussion will always ensure transparency and will motivate the people. In case of vilations,it would be a better idea to consult with the people involved to understand the situations that led to them. A blame culture or "No name - No blame" culture will not help us prevent the violations. Instead a " Just Culture" that promotes open discussion will ensure that a a fair practice is followed without compromising on the accountability.
