Oh my, been there... big time. We had a situation early last year where a certain workstream in our business just wouldn't grow. The person in charge did not manage his team properly, and we often had situations where a "well I thought YOU would do that?" situation would occur. The simple solution? A clear RACI matrix. Who's Responsible, Accountable, Consulted, and Informed. This means there's always ONE person who is ultimately accountable, even if there's multiple people being "responsible". It also helps with less "clutter commuication" as it's clear who is to be informed. We spent a couple of days and workshops to really outline ALL relevant areas of our business, and build the matrix. BIG game changer.
