First need to understand the team, the team's goal. The internal as well as external factors effecting the team's path to achieve their goal. Need to fix lots of external blockers and then start with small steps of achieving agility . Retrospect with the team, what they want and what's management's expectation from them. This can be an informal way. Also we can send out surveys, Questionnaires, and evaluate where we stand before jumping into any conclusion. Patience and communication are the key factors and all other methods are just its byproducts.
